Passports

You may apply for a U.S. Passport at the Hopkins County Clerk’s Office. Bring a completed DS-11 application, which can be found at travel.state.gov. Applications must be completed in black ink and may be typed or handwritten. Applications with white-out or scribbled out information will not be accepted. All applicants must be present to apply. You will also need the following:

Age 18 and older:

• Certified copy of birth certificate.
• Passport photo (can be made locally at the CVS or the UPS Store)
• Valid driver’s license or state issued photo I.D.
• Check or money order to U.S. Department of State for $130.00.
• Cash or check to Hopkins County Clerk in the amount of $35.

* Ages 16-17 will also need to have a parent or legal guardian present with a valid photo I.D.

Under age 16:

• Certified copy of birth certificate.
• Passport photo (can be made locally at the CVS or the UPS Store)
• Both parents present with valid, state issued I.D.
• Child must be present.
• Check or money order to U.S. Department of State for $100.00
• Cash or check to the Hopkins County Clerk in the amount of $35.
